Description: Advance care planning (ACP) has potential personal, societal and economic benefits; however, uptake remains low and variable. Any healthy, competent older person can participate in ACP, but it is particularly important for those with complex health needs. Optimal time to initiate end-of-life discussions depends on the circumstances of each individual. Patient understanding of current and future health status and their general values and goals of care should be elicited, including how the person would want decisions to be made, who they would want to make decisions and their most important priorities. Patients should be encouraged to nominate a substitute decision-maker and document their preferences in a care plan that can be shared with all members of the healthcare team, reviewed and updated regularly.
